Real life accessibility issue

I'm always thinking about accessibility when I'm building web sites - I know I don't get it completely right all the time! Quite often I come across situations off-line where a little thought about accessibility would have been helpful.

Some bright spark decide that one of the urinals at work was not working properly so took measures to stop the staff using it.

I don't think we have any blind people on our floor but I know there are some blind and visually impaired people in the building. This really is an embarrassing accident waiting to happen!
